Camper Medical
Medical forms, signed by your doctor, are required for each camper at least two weeks before the start of camp.  Medical forms are valid for three years, however a new parent signature on the form is required every year.  If you have a current medical form (signed by doctor within 3 years of the conclusion of camp) you may simply print this blank form, sign it, and staple it to your completed medical form. 

 

Medication Authorization
This form is only necessary if your child will require medication administered to them by our camp R.N.  Each medication requires a separate form.  This includes any over the counter medications that you would like your child to take during the camp day.
